Participant Forms & Expectations
CAMP Expectations and Liability Waiver: In addition to reading and understanding the CAMP Expectations, all CAMP participants are required to sign and comply to the CAMP Expectations, CAMP Liability Waiver and provide emergency contact information. To access and complete all of these, click here.
Travel: Please complete the Travel Plans form so we know how/when participants will be arriving to and departing from CAMP. This will help us ensure that we have parking permits for participants with a car on campus and are aware of travel plans for those who are flying in case there are delays/cancellations. Please return the signed form via fax (205-348-8580) or email (email@example.com) to our office by June 27, 2017.
Special Accommodations: We realize that medication and dietary needs have already been shared as part of the registration process; however, we want to make sure that there are no additional needs for us to be aware of. Please remember that CAMP team members will not administer any medications to CAMP participants. Skills Assessment: In order to appropriately place all of our CAMP participants into skill level groups, it is important that all of our participants complete this Skills Assessment. Please have this assessment completed no later than July 3rd.
Parking Permits: For participants who will park their vehicle on campus during CAMP, parking passes for the Riverside East parking lot/deck will be distributed at check-in and your keys will be collected. Vehicles must have parking passes while on campus. In order to park for the Closing Luncheon, you will need to print this parking permit.
